Outings Information Form
Use this online form to register a School or Agency with ICO and organize upcoming trips.

Hike Request Form
Use this online form to request a bus for your trip (eight weeks advance notice from the Saturday following the monthly meeting)

Trip Followup Report Form
Use this online form to report your hike (file within two weeks of hike completion: required for insurance/record keeping purposes)

Student Transportation of America Field Trip Rules and Guidelines

Field Trip Rules and Guidelines must be reviewed, read and signed by group leader and must hand to driver prior to loading the bus. This is a requirement to board the bus.

Emergency Attendance Log

Emergency Attendance Log must be filled out, this form is only in case of emergency. This form remains with group leader till or if CHP requires it, at no time is it to be given to the driver.

Use this to organize and keep track of the trips you lead (courtesy of Shirley Hickman). This is particularly helpful if you are working with multiple organizations.

Please have teachers and students complete this survey at least once a year on any trip you decide. This will provide us with data, quotes, and anecdotes to use for grant writing.
